Travellers entering mainland Tanzania now need to complete the online Traveller’s Health Surveillance Form under the Tanzania Ministry of Health Travel Advisory No.18, issued on 17 October 2026 with immediate effect. The official document tells travellers to provide accurate information before departure or on arrival and keep the confirmation QR code or reference number for port-health verification.

The requirement is a health declaration. It should be handled alongside the traveller’s visa and other entry documents rather than mistaken for an electronic visa or a replacement for immigration permission.

What changed this week

The Tanzania expands health declaration requirement explains that the mainland requirement has been expanded to all arrivals, rather than only travellers arriving from or transiting through specified affected countries. The change accompanies enhanced health screening in response to the regional Ebola situation.

The report also distinguishes targeted monitoring of people classified as high risk from a blanket quarantine for every tourist. Travellers should follow the directions they personally receive from health authorities and avoid assuming that a general news summary determines their individual screening outcome.

Prepare the form with your travel details

Use the portal named in the government advisory, AfyaMsafiri, and enter the itinerary and personal details accurately. Keep a readable copy of the confirmation available for arrival, including a way to access it if mobile data is unavailable. Where an itinerary changes, check the portal’s instructions rather than leaving inaccurate travel information unaddressed.

Treat health data carefully. A hotel message, social-media post or advertisement is not enough to establish that a third-party form is the official government process. Start from the health ministry’s published link when submitting sensitive personal information.

Check mainland and Zanzibar requirements separately

The travel report notes that Zanzibar has its own health authority and advisory. A journey involving both destinations deserves a separate review of the rules at each point of entry. Do not assume one declaration automatically satisfies every requirement on a multi-stop trip.

Before leaving, also check the latest official travel-health advice relevant to the route. If symptoms develop or the traveller receives a monitoring instruction, the appropriate health authority should guide the response.
